Only with a tight crop. At 16×16 pixels there is room for roughly one recognisable shape, so a close subject on a clean background reads and a wide scene does not. Crop before converting rather than after.

No. ICO entries are square, so a 4:3 phone frame is fitted to a square canvas. Crop to 1:1 first if you want to decide what stays in frame.

Somewhat — ICO exists for Windows application icons and favicons, where purpose-drawn artwork usually reads better at small sizes than any photograph. The conversion works fine; the design question is the harder one.
